Configuring a form

The form builder is a flexible tool designed to help you create and configure forms tailored to your needs. It allows you to organize information by adding sections and questions, each with customizable fields. Sections act as containers for questions, providing structure and organization, while questions capture the specific data inputs.

Using the form builder, you can create powerful and dynamic forms that use conditional logic, mandatory fields, and data mapping to catalog fields, making the form both user-friendly and informative. With these configurations, you can create forms that are not only structured and visually organized but also optimized for data accuracy and consistency.

Form sections

A section organizes questions into thematic or logical groups within the form, making it easier for users to understand and complete the form. Sections can be rearranged within the form using drag-and-drop functionality, allowing you to customize the form’s flow according to the intended user experience. Sections include the following fields:

  • Name - The label for the section, which appears at the top, giving context to the questions it contains.

  • Description - A brief overview of what the section covers or what kind of information it seeks.

  • Question - Each section can contain one or more questions, each with unique attributes and data capture types.

Form questions

Questions are the specific data entry points within each section. They can be set up for various data types and configurations, making the form highly versatile. Questions within each section can be rearranged using drag-and-drop, enabling you to control the sequence in which information is captured. Each question includes the following fields:

  • Name - A label for the question, clearly stating what information is needed from the user.

  • Description - Additional context or instructions related to the question to guide users in answering accurately.

  • Required - A flag indicating whether this question is mandatory. If enabled, users must complete this question to submit the form.

  • Map to catalog - A function that links the question field to a catalog field of the same type, allowing data to be standardized or referenced in other parts of the system.
