Copying a questionnaire

From the menu, select Settings.

Select Questionnaires.

The Questionnaires window displays.

From the left menu, select General Questionnaires.

Click Add New >.

From the menu, select Choose from existing.

The Existing Questionnaires window displays.

Select a questionnaire from the list.

Use the drop-down arrows to open the sections displayed as needed.

Use the checkboxes to select individual questions or entire sections to include in the new questionnaire.

When you have selected the sections and questions you want, click Use this questionnaire.

The Questionnaire Builder displays with the sections and questions you selected.

Click the Edit icon next to the questionnaire name to update the name.
